

Medical Aesthetics Consultation Policy
DeJon Salon & Aesthetics Center
(also known as DeJon Aesthetics & Wellness Center)
Medical aesthetic services at DeJon Salon & Aesthetics Center are elective and
require individualized evaluation. This Consultation Policy applies only to medical
aesthetic services provided by licensed medical professionals.
Purpose of a Medical Aesthetics Consultation
Medical aesthetic consultations are designed to:
•Review your medical history and current health status
•Discuss your aesthetic goals and treatment options
•Determine medical appropriateness and safety
•Provide professional recommendations tailored to you
A consultation does not guarantee that a treatment will be performed.
Consultation Fee
Medical aesthetic consultations with
Melissa Staricka, BSN, MN, NP-BC — Director of Aesthetics are $75.00.
•Consultation fees are due at the time of consultation
•Consultation fees are non-refundable
•When applicable, the consultation fee may be applied toward treatment, as
discussed during your visit Consultation fees reflect the time, medical expertise, and
evaluation required to safely assess treatment options.
Validity of Consultation
Consultation recommendations are valid for a limited time, as medical history
and skin conditions may change.
•A new consultation may be required if significant time has passed
•A new consultation may also be required if there are changes to medical
history, medications, or treatment goals
Medical Disclosure Requirement
Clients are responsible for providing complete and accurate medical information, including:
•Current medications and supplements
•Medical conditions or diagnoses
•Allergies or sensitivities
•Recent procedures or treatments
Failure to disclose relevant medical information may result in
treatmentbeing declined for safety reasons.
Right to Decline or Postpone Treatment
Medical aesthetic providers reserve the right to:
•Decline treatment
•Modify a treatment plan
•Postpone services
This may occur if a treatment is deemed unsafe, inappropriate, or
not medically advisable at the time of consultation.
